Loan Application Checklist (Printable)

In general, the documentation you will need includes:

Check or Credit Card to pay for Appraisal &/or Credit Report

Property Information (if you already have a contract on a house)
Sales contract including all addendums.  Copy of escrow check/Wire Receipt.
If you are selling your current home, copy of listing contract or signe contract to sell your home.
Sold a home in last 3 months? = Copy of the signed closing statement.
Names, Phone #'s of Realtors, Title Co. involved, & Ins. agents.

Income & Assets

Pay stubs for the last 30 days. (1, 2 or 4) 
  For the past 2 years:

Names, addresses & dates employed at each employer.

Copies of most recent two years of W-2's.
Statements for the most recent 2 months of retirement accounts: mutual funds, brokerage, 401k, etc.
Statements for most recent 2 months from the account you plan to bring the downpayment & closing costs monies from:
  Made any large deposits to above accounts in the last 2 months then we may need:

Explanation & source of where deposit came from.

If large deposit was a gift or is to be a gift then we need:

Signed gift letter (AmeriBest offers form - Click Here)

Copy of gift check from donor & current statement.

Copy of dep. receipt, new statement after deposited.
  If you own more than 25% of a business:

Completed & signed Federal Partnership (1065) and/or Corporate Income Tax Returns (1120) including all schedules, statements & addenda for the last two years.
  If self-employed:

Provide full tax returns for the last two years PLUS year-to-date Profit & Loss statement (please provide complete tax return including attached schedules & statements. If you have filed an extension, need a copy of the extension.)

K-1's for partnerships & S-Corp's for last two years (please check your return. Most K-1's aren't attached to the 1040.)
  If you own rental property:

Tax returns for the last 2 years & current rental leases.
  If you are retired:

Last 2 years of Pension Award Letters.
  If you receive Social Security or Disability or VA Benefits:

Last 2 years of award letters from agency or organization.
  If you are counting child support as income:

Copy of (judge signed) divorce settlement.

Copy of 12 months of cancelled child support checks or printout from County clerks office.


Full Mortgage Credit report supplied by AmeriBest Mortgage.
Explanation of credit report anomalies (FHA/VA only) including:

Late payments, credit inquiries in the last 90 days, charge-offs, collections, foreclosures, judgments and/or liens.

Bankruptcy filed within last 7 years (bring a copy of all pages of your 'discharged' bankruptcy papers).

VA Loans

Copy of Discharge Papers (DD Form 214) & Certificate of Eligibilty - AmeriBest has easy forms to assist you with this.


Drivers License or photo ID & proof of Social Security card.
Residence addresses for the past 2 years.
If applicable, a copy of your final divorce decree. (all pages) 
If you are not a citizen, copy of front & back of your green card.


Based on information appearing on your application and/or your credit report, you may be required to submit additional documentation especially after our future lender's underwriting team reviews your full complete submitted loan application.  An AmeriBest Loan Officer will be happy to assist with gathering the above items or customize a list for you.  NMLS 325885
